The steel mag catch slowly wore down over time, causing it to lose its grip on the mag during cycling.Just an FYI on the Shield S15 mag springs. I received the mags middle of Feb 2000. I also ordered the "+10%" springs. From my old 1911 days - stronger mag springs = fewer problems. I put the "extra power springs" in immediately (never used the springs that came in the mags).
